Description
*On 6/13/23, on two separate occasions, Staff A threatened children with either being sent to the office, or with having the director come into the classroom. *Staff A threatened to take away a child's stuffed animal at naptime because the child was being silly while washing his hands after lunch. *Staff A asked a child if he washed hands after using the potty. When another staff told her he did, she said to the child, "I'm watching you." *Staff A yelled to a child who was crying to stop crying or the child would get sent home.

Staff A routinely forced children to eat and forced children to nap. *Staff A required children to eat certain foods before they could eat their fruit during lunch; *On 6/13/23, Staff A withheld food from a child after repeatedly making the child ask for it; *After badgering the child to try it, Staff A forced jello into Child 1's mouth *Staff A made a child stay on his cot until all the other children had gotten up because the child didn't sleep at naptime. Staff A further denied the child a toy telling another staff the child didn't get to have it because he didn't sleep that day.

Description
Based on a complaint investigation, the following occurred on 6/13/23: *Staff A grabbed a child by the top/back of the head and yelled at the child; *During lunch Staff A repeatedly yelled a children to fix their chairs and at one point, she kicked a child's chair. *Child 3 was in a timeout and was seated at the table, crying. Staff C walked into the room, stood in front of Child 3 and growled/yelled loudly at Child 3. *Staff C came into the classroom during lunch and took food off 2 different children's plates and ate it while the children were still eating their lunch.

Description
Per staff statement and review of attendance records, on 4/19/21 one staff member was providing supervision in the Jr. Preschool room for 12 children aged 2.5 to 3 years for approximately 30 minutes, when the second staff person working in the room left for an appointment. Subsequently, the room was combined with the Toddler 2 room. The combined group of 18 children aged 2 to 3 years was supervised by one staff member from the Toddler 2 room when the staff member from the Jr. Preschool room left the group for approximately 20 minutes to deliver snack to the other classrooms.
