Potential Source Of Harm On Premises |
Areas of the outdoor play space have been separated by snow fencing and rope. The ropes and snow fencing is sagging in various areas and the snow fencing has separated from the rope in some areas resulting in the snow fencing being on the ground posing a tripping hazard.
A large commercial carpet is on the ground in the play space spanning numerous yards in length. The carpet is not flat & is folded in various areas posing a tripping hazard.
A rectangular bin is filled with dirty water that is placed near a duck that has nested in the outdoor play space. The water is accessible to children. |

An energy-absorbing surface, such as loose sand, pea gravel, or pine or bark mulch, in a depth of at least 9 inches is required under climbing equipment, swings and slides whenever the play equipment is 4 feet or more in height and in a fall zone of 4 feet beyond the equipment. At least 9 inches of mulch is not present under the monkey bars in the outdoor play space. Approximately 1-2 inches of loose mulch is observed under the monkey bars. On 5/13/21 a 5 year old child sustained a broken arm after falling from the monkey bars. |
