The under 2 play space has a 12 inch difference in surface level between the energy-absorbing surface and the grass posing a trip and fall hazard.

Staff stated they had not completed the cleaning tasks for some time. The toilet in the boys' restroom of the Preschool B class had a broken tank lid. The lid was taped.
