Child 1's parent reported the incident to Staff B later that evening and advised Child 1 had bruising to her forearms. Staff B reported the incident to Staff C who later reviewed the video footage. Staff D and Staff F wrote a counseling statement for Staff A where they cited her for violating the YMCA Code of Conduct #5 regarding abusing children. The incident was not reported to the Department until Child 1's mother contacted the Department.
